Cub Club

1. Signing your child in/out of the program.
2. Picking up your child on time - if you are going to be late, it is your responsibility to contact the program to let the staff know.
3. Providing written permission if someone other than a designated person is to pick up a student.

5. Discussing difficult issues away from the earshot or students in the program.
6. Paying fees on time.
7. Notifying the program director using THIS FORM if your child needs to be withdrawn from afterschool care.
8. Voicing concerns or compliments to the program director.

10. Information staff of a student's contagious illness.
11. Keeping your student's file/emergency information up to date.

Communication

• Communication is vital! please communicate with the Cub Club staff at any time if you have questions or concerns.
• Cub Club works with school staff (i.e. principal, secretary, and teachers) in order to provide the best, most consistent experience for your student. Information regarding your student may be shared and communicated with school staff to accomplish this.

Confidentiality

• Within Cub Club, confidential and sensitive information will only be shared with Cub Club staff that have a "need to know" in order to most appropriately and safely care for your student. Confidential and sensitive information about staff, other parents, and/or students will not be shared with parents, as Cub Club strives to protect each person's right of privacy.

• All staff members are responsible for reporting and ensuring treatment of a student's injuries. The staff will call the parent immediately to notify of any injury/incident resulting in a visible injury or head injury, complete a separate incident report and obtain the parent(s)' signature to acknowledge receipt of this information/notification. This signed copy will be kept in the student file.
• If a staff member becomes aware of any of the symptoms listed below in a student, the parent will be called immediately. Staff will call parents or emergency contacts immediately and the ill student must be picked up from the site in a timely manner. Staff will complete a separate incident report and obtain the parent(s)' signature to acknowledge receipt of the information/notification. The signed copy will be kept in the student's file.
  • The student complains of, and appears to be suffering from an illness
  • Signs of listlessness or undue fatigue
  • Fever of 100 degrees
  • Diarrhea
  • Rash
  • Pink Eye or conjunctivitis
  • Head lice, including nits
  • Vomiting

NOTE: Students should be symptom-free for 24 hours WITHOUT THE ASSISTANCE OF MEDICATION prior to returning to school and/or Cub Club.

Medication

• Medicine will not be given during the time in which Cub Club is in session. All medicine should be given to your student before or after Cub Club, or during the school day, by the school nurse.
• Parents of students requiring the use of inhalers, emergency medications, etc. will need to discuss this with the program director for proper access and storage procedures. These medications will need to be provided for Cub Club independently from those distributed to the school. Medications must be provided in the original labeled contained with instructions listed and presented to the program staff.

Pick-up from Cub Club must occur by the designated end time of the program. Late fees will be imposed as follows: $5.00 per student for each period of 1-10 minutes a parent is late. Late fees must be paid on that day. After 3 late pick-up times the student may be dismissed from the program.

• Parents are prohibited from addressing, for the purpose of correction or discipline, a student that is not their own.  If a parent should witness another parent’s student behaving in an inappropriate manner, or is concerned about behavior reported to them by their own student, it is most appropriate for the parent to direct their concern to the program director.
• It is inappropriate for one parent to seek out another parent to discuss their student’s inappropriate behavior.  All behavior concerns should be brought to the program director’s attention.  The program director will address the issue with the other parent.  Although you may be curious about the outcome of such a discussion, staff is strictly prohibited from discussing anything about another student with you.  All students enrolled in Cub Club  have privacy rights and are further protected by our Confidentiality Policy.  You may be assured that Cub Club  Staff will not discuss anything concerning your student with another parent.

Staff is required to comply with the state child abuse and neglect laws and the mandatory reporting of suspected neglect and/or abuse.  Any school official or employee acting in his/her official capacity who knows or has reasonable cause to suspect that a child has been subjected to abuse or neglect or who observed the child being subjected to conditions or circumstances that would reasonably result in abuse or neglect, will immediately make a report to the building principal or designee. 

• Every student must be signed-out of the program daily via the sign-out sheet. Only parents and authorized individuals, 16 years and older, listed on the enrollment form will be allowed to pick up a student from the program.
• Parents of a student in Cub Club are entitled to immediate access without prior notice to their student whenever they are in the care of Cub Club, as provided by law. Cub Club cannot, without a court order, limit the access of one parent by request of the other parent, regardless of the reason.
• In cases where the student is the subject of a court order, Cub Club must be provided with a certified copy of the most recent order and all amendments.
• If there is a change, a written note with the parent's signature to the Cub Club program director is required and additional legal paperwork may be required.
• Parents or designated persons will be asked to show a picture I.D. before the student is released from the program. No student will be released to leave with someone who refuses to show I.D. when asked.

Oops

Our Opportunity to Practice Skills

When your child has difficulty following our commitments, an “Oops” form will be sent home.  We understand that preschool-age children are still working on acquiring some of the skills necessary to safely navigate conflict. If your child receives an “Oops” form, please talk with your child to reinforce the skill indicated so we can work together to ensure a safe learning environment for all. 

“Oops” forms indicate how many your child has received. After receiving the third, a conference will need to be held prior to resuming after-school care. If your child receives five “Oops” forms, potential removal from after-school care may occur. 

All reasonable efforts will be made to meet the needs of the individual student; however, students may be dismissed from the program for the following reasons:

• A student has been picked up late from the program in excess of three times.
• A student demonstrates an inability to benefit from the care offered by the childcare provider whose behavior is detrimental to the other children.
• Care of a student may be discontinued if the provider and the parent(s) cannot establish a mutually satisfactory working relationship.
